Privacy policy.

Hākinakina Hauora Health Services is committed to protecting your privacy. We collect, use, store and share health information in accordance with the Privacy Act 2020 and the Health Information Privacy Code 2020.

What we collect

To provide safe, effective physiotherapy care we collect information such as your name, date of birth, contact details, relevant medical history, details of your injury or condition, and clinical notes from your assessments and treatment. For ACC-funded care we also collect the information needed to lodge and manage your claim.

How we use it

Students and whānau

Because we care for school students, we may communicate with parents, caregivers and relevant school staff (such as the sports coordinator or school nurse) about a student's injury and recovery where this supports their care. We involve students in decisions about their information in a way that reflects their age and maturity, and clinical details are only shared on a need-to-know basis.

Storage and security

Your records are stored securely in our practice management system, with access limited to the clinicians and staff involved in your care. Health records are retained for at least ten years from your last treatment, as required by New Zealand law.

Your rights

You have the right to request access to the health information we hold about you, and to ask for it to be corrected. To do so, email physio@hakinakinaservices.nz.

If you have a concern about how your information has been handled, please raise it with us first. You can also contact the Office of the Privacy Commissioner at privacy.org.nz or on 0800 803 909.
